Rates
$80 for a 50-minute session
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Is Medical Nutrition Therapy a covered service under my medical plan?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per session?
- Is approval required from my primary care physician?
- How much does my insurance pay for an out-of-network provider?
We are providers with the following insurance companies; however, being a provider does not ensure coverage.
- Bluegrass Family Health
- Humana
- Medicare (diabetes coverage only)
- United HealthCare
Payment
Cash, check, Visa and Mastercard are accepted.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
